Diltiazem 120 mg PR is primarily used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels and improving blood flow, which reduces the workload on the heart. It is also used to control certain types of fast or irregular heart rhythms, such as atrial fibrillation or atrial flutter, by slowing the heart rate. In some patients, it helps prevent episodes of angina triggered by exertion or stress.
Indications :
Hypertension (high blood pressure)
Chronic stable angina
Variant (Prinzmetal) angina
Atrial fibrillation (rate control)
Atrial flutter (rate control)
Supraventricular tachycardia (SVT)
Side Effects :
Common:
Headache
Dizziness or lightheadedness
Swelling of ankles/feet (edema)
Flushing (warm feeling)
Fatigue
Nausea
Less common but important:
Slow heart rate (bradycardia)
Low blood pressure (hypotension)
Constipation
Skin rash
Precautions :
Avoid if you have very low blood pressure
Caution in heart failure (especially reduced ejection fraction)
Not recommended in severe bradycardia or AV block (without pacemaker)
Use carefully in liver disease
May interact with:
Beta blockers (risk of slow heart rate)
Digoxin
Statins (some types)
Avoid alcohol (may worsen BP drop and dizziness)
Do not crush or chew PR tablets (must be swallowed whole)
